IEC holders are required to update IEC every year from April to June vide Notification No. 58/2015-2020 dated 12.02.2021 : 

Even if there are no changes, it is to be updated every year online. in case of non-compliance, IEC shall be deactivated. 
 

Yes, It is required to obtain IEC code for the Import and export of goods. However, as per Para 2.8 of Handbook of Procedure, Vol.-I (on the DGFT website), certain categories of importers are exempted from  IEC.

No, It is not required to file returns for IEC. IEC is required for importing and exporting of goods.
